On behalf of the University of Melbourne conference organising committee, I have great pleasure in inviting you to attend our national forum: The way forward: Chronic disease self-management in Australia to be held at the Sofitel Melbourne 26 to 27 July 2007.

Australia is embarking on one of its most significant healthcare reforms that is likely to have a direct impact on the millions of Australians with a long term chronic illness. State and territory governments across the country are implementing major policy initiatives and generating new programmes that aim to mitigate the growing burden of chronic disease.

The University of Melbourne is hosting a national conference to exchange information and initiatives relating to the operationalisation of chronic disease self-management programs across the healthcare sector.

Keynote speaker, Professor Anne Rogers, who led the evaluation of the £30 million UK Expert Patients Programme –
“a centerpiece of primary healthcare reform” will present results and implications of implementing chronic disease self-management initiatives across the healthcare continuum.

Other highlights include:
  • Exposure to Australia’s leading researchers, program developers and innovators in chronic disease management
  • Insight into policy and program initiatives in chronic disease self-management from leading state government policy makers
  • Engage in debate with Dr Norman Swan (ABC Health Report) on how best to operationalise chronic disease self-management within the Australian health care system
  • Attend innovative workshops that focus on ‘practical’ ways to develop, implement and evaluate self-management within the local Australian setting
