Chronic
condition self-management: partnerships with patients

The Flinders model of self-management care planning has evolved
over 10 years as a generic (non disease specific) method to assist clinicians
to assess, plan and motivate their clients to improve their self-management of
chronic conditions. In this workshop, Associate Professor Malcolm Battersby will
provide an introduction to the background, development and use of the Flinders
assessment and care plan tools. Case examples will be given showing how the tools
have been used and evidence provided of the effectiveness of the care planning
process in a variety of settings.
