Coordinating chronic disease self-management in the primary healthcare setting
- showcasing innovation Primary Health Branch, Department of Human Services,
Victoria.
The
Department of Human Services Victoria has taken a strong lead in
promoting self management in the health sector. Policy and funding
initiatives across both the acute and community sector have supported
the building of capacity and care delivery systems to improve self
management support and integration into chronic disease care.
This
workshop will provide an overview of the policy directions that have
created awareness, built capacity and promoted system changes to
support better responses to chronic disease particularly in the area
of self management. You will have the opportunity to hear from a
number of speakers about innovations currently being generated at
the local level through primary care partnerships and the early intervention
in chronic disease management programs.
This is an important workshop
for Victorian healthcare professionals and managers wishing to understand:
- What is needed to effectively promote self management within
an organisation- processes, training and resource allocation
- How to implement system wide changes and manage the changing
roles of health care processionals
- Strategies for planning
and affecting change at a regional level. The workshop will include
time for discussion and shared learnings.
